Privacy Policy

This Privacy Policy explains how Peter Pham Nursing Corporation ("we," "us," or "our") handles information collected through peternp.com (the "Site"). It is separate from our HIPAA Notice of Privacy Practices, which governs protected health information (PHI) collected during clinical care. If you are a current or prospective patient, please also read our Notice of Privacy Practices.
Information we collect
When you use the Site, we may collect:
- Information you give us through forms (name, email, phone, the contents of your message).
- Booking information when you click through to our scheduling tools (Black Onyx Care via Zocdoc, or other booking partners).
- Automatically-collected data: IP address, browser type, pages viewed, and time stamps, through standard server logs and Squarespace analytics.
- Cookies and similar technologies. Squarespace places cookies that help the Site function and measure traffic. You can control cookies through your browser settings.
How we use information
- To respond to inquiries you send through the contact form.
- To schedule and manage appointments, when you provide that information.
- To improve the Site's content and performance.
- To comply with legal obligations.
We do not sell your personal information. We do not use your information for cross-context behavioral advertising.
Sharing
We share limited information with vendors who help us operate the Site and the practice, including Squarespace (website host), Google (maps, analytics if enabled), and our scheduling and EHR partners. We require these vendors to protect your information and only use it to provide services to us.
Your California rights (CCPA / CPRA)
If you are a California resident, you have the right to know what personal information we collect, request deletion of your personal information, request correction of inaccurate information, and opt out of any "sale" or "sharing" of personal information (we do not sell or share your personal information for advertising). To exercise these rights, email info@blackonyx.care from the email address on file. We will respond within 45 days.
Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13.
Security
We use reasonable administrative, technical, and physical safeguards to protect information collected through the Site. No method of transmission over the internet is 100% secure.
Do Not Track
The Site does not respond to Do Not Track browser signals.
Important: do not send protected health information (PHI) through the contact form
The website contact form is not a HIPAA-secure channel. Please do not send symptoms, diagnoses, medication details, or other PHI through the form. For clinical questions, use the patient portal provided after intake, or call (714) 844-5541.
Changes to this policy
We may update this Privacy Policy from time to time. The "Effective date" above will reflect the latest version.
