Self-pay rates

| Follow-up medication management (25–30 min) | $100 |
| Extended follow-up (45 min) | $150 |
We accept payment by card and HSA/FSA at time of service. Superbills available on request for out-of-network reimbursement.
Your Good Faith Estimate (No Surprises Act)
If you are uninsured or self-pay, you have the right to a Good Faith Estimate of expected charges before your visit. See our Good Faith Estimate page for details.
Cancellations and missed appointments
Please cancel or reschedule at least 24 hours in advance. Missed appointments and late cancellations may be billed.
Refills
Routine refills are handled at scheduled follow-up visits. Refill requests outside of visits should be submitted through the patient portal at least 3 business days in advance.
